General Liability Waiver & Release of Claims**

IMPORTANT — PLEASE READ CAREFULLY BEFORE SIGNING

This Liability Waiver and Release (“Agreement”) is entered into by the undersigned participant (or the parent/guardian of a minor participant) in connection with participation in any activity, event, program, or gathering organized, sponsored, or hosted by Knightdale Baptist Church, including but not limited to: games, practices, camps, leagues, clinics, tryouts, open gyms, parades, outreach events, recreational activities, transportation to or from events, and any other church‑related or church‑affiliated activity (“Activities”).

1. Assumption of Risk

I understand and acknowledge that participation in the Activities involves inherent risks, including but not limited to: physical injury, falls, collisions, illness, dehydration, weather‑related hazards, equipment failure, and risks associated with physical exertion.

I voluntarily choose to participate (or allow my child to participate) in these Activities with full knowledge of these risks and accept all responsibility for any injury, loss, or damage that may occur.

2. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless:

Knightdale Baptist Church,

its pastors, leaders, employees, volunteers, organizers, coaches, assistants, representatives, and agents,

any partnering organizations or facility owners,

from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, disability, death, property damage, or other loss that may occur during or as a result of participation in the Activities, whether caused by negligence or otherwise.

3. Medical Treatment Authorization

In the event of an injury or medical emergency, I authorize Knightdale Baptist Church and its volunteers to secure medical care deemed necessary for myself or my child.

I understand that I am solely responsible for all medical expenses incurred.

4. Personal Responsibility

I agree that:

I (or my child) will follow all rules, instructions, and safety guidelines provided by church staff or volunteers.

I am responsible for ensuring that I (or my child) am physically able to participate safely.

I will not hold the church or its volunteers responsible for lost, stolen, or damaged personal property.

5. Photography & Media Release (Optional but Common)

I grant permission for Knightdale Baptist Church to use photographs, video, or audio recordings of myself or my child for ministry‑related purposes such as promotion, social media, or printed materials.

(If you prefer to opt out, please notify a leader in writing.)

6. Consent for Minors (if applicable)

If the participant is a minor, I certify that I am the parent or legal guardian and have full authority to sign this Agreement on their behalf. I accept all terms of this Agreement for the minor participant.

7. Acknowledgment of Understanding

By signing below, I acknowledge that:

I have read and fully understand this Liability Waiver and Release.

I am signing it voluntarily and without coercion.

I understand that this document affects my legal rights.

I agree to be bound by its terms.
